                FAMILY PHYSICIAN-BRIDGE STREET FAMILY MEDICINE
SAUGERTIES, NY
 
Since 1992, Bridge Street Family Medicine-a comprehensive Primary Care (CPC) participating practice has been providing high quality primary health care to the Hudson River Valley communities of Ulster, Dutchess and neighboring counties. This year, Bridge Street will be the medical home for more than 7,500 patients, with three physicians and one physician assistant caring for all age groups from infants to the elderly. Establishing a relationship built on trust and respect is a core component of our patient-centered family practice. Our team of practitioners, nurses, medical assistants and care coordinators and fron desk facilitators work together to ensure a program integrating care management that leads to healthier individual, families and communities.
 
Bridge Street Family Medicine is seeking a Family Physician for its private practice in Saugerties, New York. The Family Physician will report to Laura Decker, MD. The physician will lead a multidisciplinary care team.
 
The Town of Saugerties is located within Ulster County, about 90 minutes by care north of New York City and uniquely part of two spectacular geographic regions: The Hudson River Valley and the Catskill Mountains. Offering the beauty of a four-season landscape, Ulster County boasts some 350 miles of nature trails for hikers, bicyclists and cross-country skiers, magnificent mountain views and rock climbing challenges. It is also the center of the arts and culture in the Hudson Valley, including galleries and outdoor theater and art fairs, and just a few miles from the Fisher Center for the Performing Arts as Bard College offering year-around performances in dance, opera, music and theater.
 
A tour of Saugerties takes you on a journey of family-friendly neighborhoods, multiple baseball fields and multi-generation, family-run stores. A weekly farmers’ market makes available the wonderful local produce for which the area is renowned. Just 10 miles from Saugerties, the City of Kingston offers excellent schools, high-end residential communities, quality health and hospital facilities and a rich history complete with revolutionary War re-enactments. The historical sites of the Roosevelt and Vanderbilt estates are a 20-30 minute drive. Kingston has a deep water dock making it an ideal location for river-boating.
 
RESPONSIBILITIES
  • Provide high quality primary care to bridge Street Family Medicine patients from infant, pediatric, adolescent adult and geriatric groups;
  • Lead a multidisciplinary care team;
  • Provide comprehensive patient care and establish a family partnership whenever possible to ensure the needs of the patients are met on an ongoing basis
  • Ensure that high quality and cost-effective primary care is consistently delivered
  • Provide appropriate specialist referrals and follow-up when indicated
  • Maintain electronic medical records in compliance with Bridge Street Family Medicine standards
  • Engage patient, family and care teams in sustaining end enhancing the health and wellbeing of the patient
  • Maintain familiarity and ongoing research and development in relevant fields
  • Ensure performance improvements initiatives are supported with timely and accurate data and consistent actions
  • On-call once every four days and once every four weekends; average calls per night: Less than one.
 
REQUIREMENTS
  • Doctor of Medicine or Osteopathy; licensed in New York State prior to start date;
  • Completion of there-year family practice residency
  • Board-eligible from American Board of Family Medicine; board certification required within two years after completion of residency training;
  • A minimum of one year experience in the practice of family medicine;
  • Outstanding “bedside manner”; patient-centric with empathy, excellent active listening and communication skills; engaging and efficient;
  • Outstanding clinical capabilities and judgment
  • Focused on collaborative, team-based care
  • Responsive and comfortable working in a high accountability environment
  • Proficient in HER systems
  • Flexible
  • Excellent  judgment and ability to prioritize resources where they will be most useful
  • Comfortable receiving and providing constructive feedback;

Open Door is seeking a Family Physician for its Family Medical Center in Brewster, NY. The Family 
Physician will report to the Medical Site Director and will be based in Brewster,
NY. He/she will lead cross-functional care teams.  All Open Door Family Medical Centers are located 
in Westchester and Putnam counties located just North of New York City, which offer excellent 
schools, easy access to a multitude of outdoor activities, and superb transportation connections to 
New York City.

RESPONSIBILITIES

• Provide high quality primary care to Open Door patients of infant, p1;;diatric, adolescent, 
adult and geriatric groups
• Lead cross-functional care teams
• Provide comprehensive patient care and establish a family partnership whenever possible to 
ensure the needs of the patients are met on an ongoing basis
• Ensure high quality primary care is delivered cost-effectively, consistently
• Provide appropriate specialist referrals, follow-up and hospitalization when indicated
• Maintain electronic medical records in compliance with Open Door's standards with high 
efficiency
• Engage patient, family and cross-functional care teams in sustained health and well- being of 
the patient
• Partner with Open Door's Behavioral Health team and resources
• Maintain familiarity with ongoing research and development in relevant fields
• Ensure performance improvement initiatives are supported with timely and accurate data and 
consistent actions
